Johnstown Pointe Nursing and Rehabilitation, a Foundations Health Solutions Care Center, is now open in Johnstown, Ohio.  This 80 room nursing home boasts private rooms with baths and Foundations Health Solutions quality management and clinical care. Johnstown Pointe joins seven sister facilities in Columbus, all serving their demographics with varied and quality service lines. These facilities include:

Crown Pointe Care Center- This lovely center concentrates on not only short term and long term rehab, but also caring for our veterans with a VA contract and memory care diagnoses.

Highbanks Care Center- specializing in behavioral and mental health. This center is completely secure to lessen the anxiety of family members and keep our patients safe and comfortable.

Trueman Pointe Nursing and Rehabilitation- a center that concentrates on peritoneal dialysis, long term care, and higher acuity patient rehab. This center also recognizes a patient’s right to smoke and provides scheduled and safe smoking.

McNaughten Pointe Nursing and Rehabilitation- this center has a known track record in caring for ventilator patients and also services a dementia clientele.

Meadow Grove Transitional Care- A striking state of the art center that specializes in transitioning patients home post hospital stay and in patient rehab. This center boasts all private rooms and baths.

Capri Gardens Rehabilitation and Nursing Care- Our newest building in Columbus is a beautiful care center with private rooms and baths focusing on rehabilitating patients in the Lewis Center area.

Darby Glen Nursing and Rehabilitation Center- This center is one of two Foundations centers in Hilliard, Ohio. Darby Glen concentrates on quality short term rehab and long term care.
